The Ventura County Medical Examiner released Naya Rivera's autopsy and toxicology report, revealing her tragic final moments. Her mother, Yolanda Rivera, also detailed what went wrong at Lake Piru in California, which ultimately led to her demise.

Yolanda revealed her daughter and her grandson, Naya's child Josey Dorsey, was at Lake Piru for a barbecue in July. However, the Glee star had a change of mind and decided to rent a pontoon boat instead, In Touch noted.

While on the lake, the two went for a swim. Josey, Naya's kid with former husband Ryan Dorsey, said he and his mom counted one to three before they jumped off the boat together. After they jumped in the water, the singer-model told his four-year-old son to get back on the craft.

She helped him climbed the boat, leaving herself in the water. Once Josey was safe and secured, he heard his mother asking for "help" and saw her putting her arm in the air. After a while, she disappeared into the lake.

Yolanda told the police Naya knew how to swim and was also familiar with Lake Piru. She didn't have any medical conditions, only vertigo, which could be worst when she's in the water.

She had vertigo treatments at Cedars Sinai and was able to control her symptoms, which included vomiting, by taking antihistamines. Yolanda also revealed Naya had a prescription drug, Adderall, to cure her anxiety.

However, she's not sure if her daughter was taking it. Naya was undergoing sinus infection treatment at the time of her death, although the medication was unknown. Also, she had no known recent falls or injuries before her demise, nor she even had any suicidal tendencies.

The toxicology report also revealed she only had low levels of "ethanol, caffeine, diazepam, nordiazepam, amphetamine, and phentermine" in her system when she died. Her blood alcohol concentration then was at 0.016.

Hence, according to People, her cause of death remained drowning, and its manner was "ruled as an accident." Investigators confirmed she died on July 13, five days after she disappeared. She was 33.

Naya was reported missing when she didn't return the boat she rented on time. The lake's stuff then found it at the north side of the lake, with only Josey on board, wearing his life jacket and sleeping.

One adult life jacket, believed to be Naya's, remained on the boat. A day after, police announced she's presumed dead, considering her disappearance as a "horrible accident."
